Documents Required for Child Care Fee Assistance |

You
MUST bring all of the following documents that apply to your circumstances to the Intake Eligibility Iinterview. Eligibility for child care fee assistance is determined based on verified information only.

Identification |
- Social Insurance Number (SIN card) for adult family members.
- Birth Certificates for all family members or Immigration papers or Citizenship Cards for all family members.
- Separation Agreement/Divorce papers.

Income Verification |
- Income Tax Notice of Assessment.
- Canada Child Tax Benefit Notice.
- Handicap/Disability Documentation.
|
|
Employment Verification |
- 2 recent, consecutive pay stubs.
- A letter from your employer, if you are returning to work or starting new employment. The letter must indicate days/hours of work, start date and rate of pay.
- Record of employment (if applicable).

| Students Verification |
- OSAP Assessment Summary Form (if applicable).
- School registration and time table.
|
| Self-employed Application |
- Business registration forms.
- Most recent Business and/or Personal Income Tax Returns.
- Income Tax Notice of Assessment.
