Client Intervention and Support Services

A service to provide social work support to older adults in the privacy of their own homes.

What can Client Intervention and Support Services do for you?

  • In-home assessment provided by a social worker
  • Supportive and instrumental counselling provided for individuals and their families regarding housing, alternative accommodations, finances, bereavement, family dynamics and long-term planning
  • Information and referrals to community programs and services
  • Advocacy provided to maximize individual well-being
  • Crisis intervention as needed
  • Short-term and long-term case management available

Who can use this service:

  • Seniors 60 years of age or older who live in the York Region community
  • Family members or caregivers

Is there a cost for this service?

No. The Regional Municipality of York, Health Services Department, Long Term Care & Seniors Branch and the Ontario Ministry of Health and Long-Term Care provide funding for this service.

Adult Day Programs

The Regional Municipality of York operates Adult Day Programs at the Maple Health Centre and Keswick Gardens. These programs provide a supportive environment to assist individuals residing in the community to achieve and maintain a maximum level of functioning as well as provide relief for families and caregivers.

The Adult Day Centre Program offers a social and therapeutic program during the day in a comfortable home-like setting. A variety of activities such as fitness, gardening, reminiscence groups, music therapy, creative arts and crafts and pet therapy are provided.

Some programs are available up to 6 days per week. Some programs include lunch and snacks.

A daily fee is charged based on a client's income (up to a maximum of $20.00 per day). Transportation may be available.

Maple Health Centre Adult Day Programs are available for individuals with: Alzheimer's or other related dementias, physically disabled, acquired brain injured and those coping with communication disorders.

An adult day program for individuals with Alzheimer's or other related dementias is offered in partnership with the Alzheimer Society of York Region and is available 5 days per week at Keswick Gardens and Maple Health Centre. For admission information about any of the Day Programs please contact the Adult Day Program Co-ordinator at the York Region Community Care Access Centre (CCAC) at 1-888-470-2222.

Physically Disabled

This program provides individualized physical, social and recreational activities for people with physical disabilities. The program runs Mondays and Thursdays at the Maple Health Centre.

Acquired Brain Injured

This program operates Tuesdays and Fridays at the Maple Health Centre. The program is designed to meet the needs of individuals with acquired brain injury as a result of a traumatic event or other non-progressive neurological diseases.

Communication Disorders

This program operates on Wednesdays at the Maple Health Centre. The program is designed for adults with aphasia and related communication disorders due to stroke, brain injury or brain disease. The program is offered in mutual aid format, facilitated by speech/language staff and trained volunteer "conversation partners". A multi-modal approach including augmentative and alternative communication is used. Sessions include adapting to communication loss, literacy and computer work, creative communication, problem solving and family support. Regional Psychogeriatric & Mental Health Consulting Service

The Psychogeriatric Consulting Service supports Long Term Care Facilities in York Region and Community Support Agencies that serve people who exhibit difficult or aggressive behaviours. Consultants operate in a continuum of care model and serve as a link between inpatient, specialty and community resources, among others.

This service is located at the Newmarket Health Centre.

Alzheimer Resource Centres

Alzheimer Resource Centres contain reference materials, books and journals that relate to Alzheimer's Disease and other dementing illnesses. The Centres are also equipped with computers that provide families and caregivers with Internet access to a wide range of current information on Alzheimer's Disease.

Community Alzheimer Resource Centres are located at the Maple Health Centre and Keswick Gardens. The Centres are available for community clients, families and caregivers of both Day Centre clients and Long Term Care Facility residents. The Resource Centre at Keswick Gardens is open Monday to Friday from 7:30 a.m. – 5:30 p.m. and at the Maple Health Centre from 8:00 a.m. – 8:00 p.m.

Meals on Wheels

The Long Term Care & Seniors Branch prepares meals for the Community Home Assistance to Seniors (CHATS) Meals on Wheels Program. Volunteer drivers pick up the meals and deliver them to program member homes throughout York Region.

Personal Emergency Response System

Emergency Response SystemA personal emergency response system (P.E.R.S) is a product that enables clients to summon help in a crisis situation. The system is monitored 24 hours per day, seven days per week by a central monitoring station. The station starts the process by checking into problems and/or alerting help when an emergency signal is received.

Seniors Community Programs of the Regional Municipality of York provides a subsidy for the purchase of a P.E.R.S. The subsidy is available to clients who need financial assistance and who meet the eligibility criteria.
