The Regional Municipality of York, Health Services Department, Long Term Care & Seniors Branch currently operates one 100 bed Long Term Care Facility – the Maple Health Centre and one 132 bed Long Term Care Facility, the Newmarket Health Centre. Approximately 600 staff provide services to promote the health, well-being, safety and independence of individuals who can no longer live independently.
The Newmarket Health Centre and the Maple Health Centre are not-for-profit facilities funded by the Ministry of Health & Long Term Care and the Regional Municipality of York. The Long Term Care Centres provide health services to people 18 years of age or older who are no longer able to live independently in their own homes and require nursing and personal care. Special emphasis is placed on meeting the needs of the difficult to place/ hard to serve individuals with heavy, complex, physical and/or psychiatric care requirements. We also provide care for individuals who have a high degree of cognitive impairment and a loss of intellect, affecting their ability to care for or protect themselves. A comprehensive continuous quality improvement (CQI) and risk management program is provided to ensure the provision of high quality care and services. Short stay/respite programs are also available at the Newmarket and Maple Health Centres. The Newmarket Health Centre has 2 complex care units, 2 secured care units and 3 short stay/respite beds. The secured units have access to a secured courtyard with gardens and walking paths, providing a safe environment for individuals with cognitive impairments who may wander. The Centre also has a Convalescent Care Program which offers intensive rehabilitation services to people who need additional time to recover strength, functioning & independence following illness or injury. The Centre also provides office, educational and programming space to a number of community-based health and social services agencies. Meeting rooms are also used by community groups. Regular Family Support Group sessions are open to both families of residents and families of individuals in the community on the Centre's waiting list. At the Newmarket Health Centre, residents can choose from preferred accommodation or basic accommodation. Facility rates are available. Maple Health Centre

The Maple Health Centre is a modern facility that opened in 1998. It is designated by the Ministry of Health & Long Term Care (MOHLTC) as a class "A" facility. This designation means that it meets or exceeds all current MOHLTC design standards. The Maple Health Centre is located in the growing community of Vaughan. Residents can choose from preferred accommodation or basic accommodation, Facility rates are available. The Centre provides residents with an aesthetically pleasing, accessible, supportive, home environment. Picturesque courtyards, therapeutic gardens and designated walking paths are available for all residents. Perimeter security consisting of coded electromagnetic locks, alarms, security cameras and anti-wandering bracelets are also in place to ensure the security of residents inclined to wander. The Maple Health Centre offers facility programs including Psychiatric Care and Cognitive Care in secured units and Complex Care on two small open-concept units. In addition to its 100 long term care beds, the Maple Health Centre has two Day Centres, a Short Stay Program, and a Community Alzheimer Resource Centre. In addition meeting space is available for community groups. Facility Program and Support GroupsThe Long Term Care Facility Program embraces a multidisciplinary team approach to providing care to residents. The team includes nurses, physicians, personal support workers, physiotherapists, pharmacists, personal care aids, volunteers, chaplains, dieticians, social workers, and housekeepers. Each discipline works together to design a specific plan to meet the needs of every individual. Volunteers provide assistance with special events, the Tuck Shop, outings and unit based activities. Family involvement is encouraged and supported at all times. Therapeutic and Recreational Programs A full-time Program Co-ordinator plans both large, Centre-wide activities and smaller unit-based activities.
A Snoezelen Therapy room and a portable snoezelen cart are available at each Centre for resident and family use. These provide an atmosphere of sensory stimulation and relaxation. The Centre Social Worker provides counselling services to individuals applying for Long Term Care and to recipients of long term care services. Social Work Services also assist clients and their families to identify alternative courses of action and to obtain the most appropriate services available for their needs. The goal is to assist clients to meet their psycho-social needs and to maximize the clients' quality of life in accordance with Gerontological and Social Work principles and ethics. Social Workers are active both in the pre-admission and post-admission phases of placement, conducting tours and giving information about the Centre. The Social Work Program supports a continuum of care philosophy by working with community agencies and health care facilities, making referrals where appropriate and maintaining follow-up when indicated. At each Centre, the Social Worker facilitates a Family Support Group for family members of residents receiving Complex/Heavy Care and families of individuals on the Centre's waiting list who are still living in the community. Complex/Heavy Care Support Groups Both the Newmarket Health Centre and the Maple Health Centre have established a Family Support Group for the next of kin of residents receiving Complex/Heavy Care and families of individuals on the approved waiting list. The Newmarket Health Centre group meets on the 4th Thursday of the month from 7:00 p.m. 8:30 p.m. The group is facilitated by a Social Worker who can be reached at 905-895-2381 ext. 3405. The Maple Health Centre group meets on the last Monday of the month from 7:00p.m. 8:30 p.m. in the Town Hall. This group is facilitated by a Social Worker, who can be reached at 905-303-0133 ext. 3637. The sessions address a variety of topics such as feelings regarding placement, sharing ideas with others in the same situations and providing the comfort of discovering "I'm not the only one. Participants believe that being part of a family support group gives them a sense of community and belonging in addition to coping strategies. Alzheimer and Related Disorders Support Group Individuals affected with a severe cognitive or psychiatric impairment are not the only ones that suffer as a result of the disease. Coping and dealing with the losses associated with having a loved one diagnosed with a dementing illness and his/her subsequent placement in a Long Term Care Facility can be very difficult for family members. To help support families in this regard, monthly support groups are conducted, free of charge, for the families/caregivers of residents of our cognitive care units and for the families/caregivers or applicants on the approved waiting list. These monthly family support groups are conducted in partnership with the York Region Alzheimer Society. Staff from the Alzheimer Society facilitate the groups at both the Newmarket and Maple Health Centres.
