NutriSTEP® Questionnaire

The NutriSTEP® questionnaire has 17 questions that ask parents about eating and other habits that can influence their child's nutrition. The questions have several responses. Parents choose the best response option to describe their child's behaviour.

The NutriSTEP® questionnaire takes approximately five minutes for parents to complete. After a parent completes the questionnaire, they add up the total nutrition risk score.

There are three levels of nutrition risk scores with guidelines for parents:

  • Score of 20 or less: The child's eating and activity habits are good. There may be items that parents want to work on. Parents are encouraged to read the nutrition education fact sheet that is provided with the questionnaire for tips and more information
  • For Score of 21 to 25: The child's eating and activity habits can be improved by making some small changes. Parents are encouraged to read the nutrition education fact sheet that is provided with the questionnaire or go to resources for tips and more information. Parents can also speak to a registered dietitian at EatRight Ontario (1-800-510-5102).

  • Score of 26 or greater: The child's eating and activity habits can be improved by making some changes. Parents are encouraged to talk to a health professional, such as their family doctor or a registered dietitian.
