Vaccine  Storage, Handling and Ordering

All publicly funded vaccines for Ontario are available to physicians by faxing an order to York Region Community and Health Services at 905-895-6066.

When you are a physician opening a new practice in the Regional Municipality of York
Prior to ordering any vaccines, each facility must have an office visit by a nurse on the Vaccine Preventable Diseases team, who will install a digital minimum-maximum refrigerator thermometer and provide instructions on the storage and handling of vaccines. Once the visit is complete, the office must fax in proof of at least three days' temperatures within the acceptable range of +2° to +8° C before an order can be processed. Each subsequent vaccine order must be accompanied by a temperature log sheet.

If vaccine refrigerator temperatures go outside the range of +2° to +8° C, the office must contact the Vaccine Preventable Diseases team immediately at 1-877-794-1880 and press 9 to speak with a cold chain nurse.

If you have any general enquires about the vaccine order form, please call the Vaccine Information Line at
905-830-4444 or 1-877-464-9675, Ext. 4033.
