Vaccine Storage, Handling and Ordering All publicly funded vaccines for Ontario are available to physicians by faxing an order to York Region Community and Health Services at 905-895-6066. When you are a physician opening a new practice in the Regional Municipality of York If vaccine refrigerator temperatures go outside the range of +2° to +8° C, the office must contact the Vaccine Preventable Diseases team immediately at 1-877-794-1880 and press 9 to speak with a cold chain nurse. Immunizations While severe side effects following an immunization rarely occur, physicians have a duty to report any adverse events promptly. To report a vaccine-associated adverse event, complete a Report of Adverse Events Following Immunization (AEFI) Form and contact York Region Public Health Vaccine Preventable Diseases Unit at 905-830-4444, ext. 3452. York Region Community and Health Services offer regularly scheduled community immunization clinics where both mandatory and voluntary vaccines are available to York Region school-aged children.
