           Regional Insurance Certificate Form

The Region's Insurance and Risk Management section is responsible for protecting the Region's assets from accidental loss utilizing various methodologies including risk analysis, loss prevention, risk finance and risk transfer.  When vendors, contractors and service providers enter into a contract with the Region, proof of insurance is required before commencement of work or services using the Region's Insurance Certificate Form.  This form can be printed, executed  and faxed to the Insurance and Risk Manager, Policy Risk and Treasury Branch of the Finance Dept.at 905-895-6079.
