Ambulance Patient Record Release
York Region keeps records (called Patient Care Reports) of interactions they have with their patients. Patients and their lawyers or insurance companies can request these records. Records cannot be released without the signed consent of the patient.
A record will include notes from the time that the ambulance arrives until the time that the patient leaves paramedic care.
Cost
This service is free of charge for patients requesting their own information. Law firms and insurance companies must pay a fee of $30 for this service.
Please Note: If York Region staff are unable to find a report, they will return your cheque to you.
How to Request a Report
Fill out an Authorization to Disclose Personal Information and send it by mail.
The Authorization to Disclose Personal Information must be filled out by the patient.

Law Firms and Insurance Companies
Send us:
- A letter on company letterhead with the following information:
- Name of the patient
- Patient’s date of birth
- Date, time and location of the incident
- Name of the hospital that the patient was taken to (if applicable)
- Law Firm file number
- Any other details that may be relevant
- The signed Authorization to Disclose Personal Information form
- A cheque for $30 made out to "The Regional Municipality of York” for each form required
