Personal Health Information Protection Act
The Community and Health Services Department maintains the confidentiality and privacy of your personal information. The department collects, uses and discloses your personal information responsibly and with compliance with the Personal Health Information Protection Act, 2004.
The Personal Health Information Act aims to:
- Establish rules for the collection, use and disclosure of personal health information that protect an individual’s privacy
- Provide individuals with a right of access to personal health information about themselves
- Provide individuals with rights to require the correction or amendment of personal health information
- Provide for independent review and resolution of complaints
In accordance with the Personal Health Information Act, York Region Public Health has become a user of the ConnectingOntario electronic health record system. ConnectingOntario:
- Is overseen by eHealth Ontario with the goal of improving the health care you receive by making your health records available on a secure electronic format
- Enables your medical records to be shared among family doctors, hospitals, long term care homes and home and community care services
- Limits access to your health records to only those health care providers who need to care for you

According to the Personal Health Information Act, a fee may be charged for providing access to an individual’s records of personal health information.
