Partner Engaged Assessment of Community Health
Thank you for accessing the Partner Engaged Assessment of Community Health (PEACH) survey.
This survey is for physicians, primary care providers and community organizations providing health and social services to the population of York Region. We value the unique knowledge that you have regarding the health of patients, residents and clients in York Region. This is a voluntary survey and will take approximately 10 to 15 minutes to complete.
We commit to sharing the survey results from the PEACH Project with you so we can all gain a better understanding of how we can meet the needs of our residents, patients, and clients.
If you work or lead a community organization, such as a community group, service provider or hospital, this is the survey for you.
