Mental Health Consultation for Seniors

Please be advised that as of April 1, 2019, the Integrated Psychogeriatric Outreach Program (IPOP) transitioned to a new health service provider, LOFT Community Services.

LOFT has over 30 years of experience providing specialized, community-based support services to older adults, including seniors with complex challenges such as dementia, addiction and mental health and behavioural issues; making them an ideal IPOP provider.

This service is available to York Region residents who are 60 years or older with mental health concerns.


This service is provided at no cost.

How to Apply

Referrals to this program are accepted from medical doctors (general practitioners and specialists), health professionals/community agencies and through self-referral with general practitioners’ support and agreement.

Referrals can be submitted by contacting 905-955-2413 or by faxing referral forms to 1-844-834-4638.
