Child care centres must register with the Ministry of Education prior to submitting this notification form to York Region Public Health. For more information please visit: Apply for or renew a child care licence | ontario.ca

New child care centres must inform York Region Public Health of their intention to operate in accordance with the Health Protection and Promotion Act, R.S.O 1990, c. H. 7 and O. Reg. 493/17. York Region requires at least seven (7) business days’ notice prior to beginning operations.

Completing the notification form allows the public health inspector to review your file and schedule a pre-opening inspection.

The personal information submitted in this form is collected under the authority of the Health Protection and Promotion Act, R.S.O 1990, c. H. 7 (HPPA) and in accordance with the Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c.M.56. and will be used for the purpose of reviewing the new child care setting's compliance with the HPPA.
