Notification of Disinterment and Body Shipment

Notification of disinterment must be made to the local Medical Officer of Health in compliance with the Funeral, Burial and Cremation Services Act, 2002 S.O. 2002, c. 33. Corpses with certain communicable diseases must be managed in accordance with the requirements of the Health Protection and Promotion Act, R.S.O. 1990, c. H.7. and O. Reg. 557: Communicable Diseases, including the required approvals of the Medical Officer of Health.

Please complete and submit this notification and application form to York Region Public Health at least five business days before the disinterment and/or body shipment.

The personal information on this form is collected under the authority of the of Health Protection and Promotion Act, R.S.O. 1990, c. H.7 and R.R.O. 1990 Reg. 557: Communicable Diseases – General and the Funeral, Burial and Cremation Services Act, 2002 S.O. 2002, c. 33. York Region Public Health will use the information collected under this form to prevent, eliminate and/or reduce the effects of a health hazard during corpse management and to issue the required document(s) to the applicant.
