If you use the dashboard, we invite you to take part in this survey. Your feedback is important because it will help us improve the dashboard and make sure it meets the needs of our partners across the Region. The survey will take about 2 minutes to complete.
Taking part in this survey is your choice. Your answers will be anonymous. You can stop the survey at any time, but we cannot remove your answers after you submit them. Your decision will not affect any services you get from York Region Public Health.
