York Region is conducting a survey to determine residents’ awareness of the potential harms and risks associated with vaping. We are inviting all York Region residents to participate in this survey. Your experience and feedback are valuable as the results from this survey will help us enhance our Vaping programing and interventions and determine the success of our Youth Vaping Awareness Campaign, York.ca/VapeFacts. Responses collected for residents over the age of 18 years will be used to inform future program planning.
Your Participation
- Participation in this survey is voluntary and your responses will remain anonymous and confidential
- You may choose to withdraw from the survey at any time by closing the web browser; however, we will not be able to remove your responses once you have submitted the survey
- Your decision to participate will not affect your relationship with York Region
- Aggregated survey results will be combined for reporting purposes
- Information will only be used to inform future program planning
This survey will take approximately 5-10 minutes to complete and will remain open until September 30, 2026.

- There will be a draw for a $100 Walmart gift card following the end of this survey period
- To be entered in the draw please follow the link provided on the page shown once you have submitted your response. This will open a separate submission form that will allow you to be entered in the draw
- This will not compromise the anonymity of your survey submission
- Participation in the draw is voluntary
- The contest is open to individuals who live or work in York Region and who have completed this Vaping Behaviour and Awareness in York Region Survey
- Individuals under the age of 16 who wish to enter the contest must be entered by their parent or legal guardian on their behalf
The random prize draw will take place on October 5, 2026. The winner will be contacted via the e-mail address provided on the electronic ballot. If no response is received within 1 week of the random prize draw, a new winner will be drawn at random until the prize is claimed. One entry per person.
Notice of Collection
The personal information submitted in this form is collected under the authority of section 7 of the Health Protection and Promotion Act, RSO 1990, c H.7 and in accordance with the Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c M.56, and will be used for the sole purpose of contacting you regarding the Vaping Behaviour and Awareness in York Region Survey contest.
