If you or someone you know had a bad reaction to street drugs (unregulated supply) including an overdose or drug poisoning, you can make an anonymous and confidential report to York Region Public Health and potentially help save someone’s life.
You may choose to withdraw from providing information at any time; however, we will not be able to remove your responses once you have submitted the tool. Your decision to participate will not affect the services you receive from York Region Public Health. This tool will take approximately 8-10 minutes to complete.
