Affidavit in Support of a Request for Reopening (POA 0801) If you missed your trial date you may apply to the court to reopen the case. There are three forms that must all be completed. The Affidavit can be sworn or affirmed at the court office.

Application for Extension of time to Appeal (POA 0206) If it has been longer than 30 days since you were convicted please fill out this form to ask the Court to extend your time to appeal.
Application for Extension of Time to Pay Fine Use this form to request more time to pay your fine.
Application to File Appeal without Paying the Fine (POA 0226) If you cannot pay your fine before appealing, please fill out both forms as part of your appeal package.
Application to Justice to Strike Out Conviction Within 15 days after becoming aware of the conviction, you can use this form to make an application to a justice to strike out the conviction if you believe that the certificate of offence is defective as established by O. Reg. 472/21
Certificate of Striking out Conviction (POA 0827) If you missed your trial date you may apply to the court to reopen the case. There are three forms that must all be completed. The Affidavit can be sworn or affirmed at the court office.
Change of Address Form Please list all ticket numbers and submit to the court noted on your ticket or trial notice. Courts are NOT notified by the ministry of your address change.
