Animal Observation Period Video Health Assessment

The Information requested is collected under the authority of the Health Protection and Promotion Act and in accordance with the Municipal Freedom of Information and Protection of Privacy Act.  The information will be used to assess your animal’s health and will be maintained as part of the investigation.  The information will be used assist in the prevention of rabies and program evaluation.

I affirm that my animal is healthy at the end of the observation period

To ensure we are able to assess the animal's health please ensure:

  1. Only the animal under observation is in the video
  2. The video is recorded in good lighting
  3. The animal is moving
  4. The animal is within 10 feet of person recording the video
  5. At the beginning of the video say the date and time of the recording
  6. The video is approximately 10 seconds in length
  7. The video is between 50-100 MB
