This is a resource for health, early learning and child care professionals in York Region who wish to refer a young child or their family for support services. It provides information about programs and services available including information regarding eligibility and referral.

Infant/Child Development Services

Supports families who have infants and young children from birth to school entry who have been diagnosed with, or are at risk of having a developmental delay.

Services are offered primarily at home or in community settings and York Region offices and include:

  • Home visits with families
  • Routines-based interview to determine areas of needs
  • Child development screening and assessment
  • Family service plans
  • Coaching parents to support their children’s participation in all aspects of daily life
  • Neonatal follow-up clinics for children born prematurely. (Neonatal follow-up clinics require a doctor’s referral)
  • Developmental monitoring for children at risk of delays
  • Activities and play groups that teach children new skills and promote development
  • Workshops and support groups for parents and caregivers
  • Referrals to community agencies
  • Transition to school support
